
VOLUNTEER INFORMATION
As with all Pop Warner associations, the Blizzard association relies on volunteers to provide the coaching and support for the players. Below are the necessary items for the volunteer application process:

2018 Volunteer Application – All volunteers (coaches, team parents, game day helpers, etc.) must complete and submit a volunteer application. The information in this application will be used to a) gain board/team approval for the vounteer and b) initiate a background check (criminal and sex offender).

Proof of Identity – A copy of current (non-expired) drivers license, passport, or military ID are acceptable forms of proof of identity

PROCESS FOR VOLUNTEER SUBMISSION:

1) APPLICATION: Complete and sign the volunteer application
2) SEND: Email COMPLETED volunteer application AND copy of proof of identify to monumentblizzard@gmail.com
IMPORTANT: the subject line of the email should be in the following format:
"Volunteer - FirstName LastName - Blizzard Team Name" where Blizzard team name is either TinyMite, MiteyMite, Junior PeeWee, PeeWee, or Junior Varsity.

3) The association president will contact you within 7 days of your submission.
